LPN with Home Health team in the Saginaw area

Reverence Home Health and Hospice, an Ascension at Home company, a premiere home health organization has an exciting opportunity for a full time LPN in the Saginaw area!

At Ascension Health at Home, our LPN’s have a direct impact on the lives of patients within your local community. 

POSITION SUMMARY:

Primary function is to administer skilled nursing care, under the supervision of a registered nurse, for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family, and referring agency.

MINIMUM QUALIFICATIONS:

Education/Licensing/Certification:

• Graduate of an accredited school of Practical nursing
• Current State License as a Licensed Practical Nurse
• Current Driver’s License
• Current CPR, negative TB screen and Hepatitis consent/declination

Experience:

• Minimum 2 years’ experience as an LPN
• Home Health experience; beyond private duty
• Experience with patient scheduling; scheduling patient services and coordinating with staff
• Strong customer service experience

Knowledge and Skills:

•  Good interpersonal and organizational skills required.
• Excellent computer skills to include Microsoft Office and Outlook.
• Knowledge of medical terminology
• Strong observational and assessment skills
• Fast learner
• Strong problem solving skills and creative thinking
