    Hospice Aide (Full-time)

    Seasons of Life Hospice House - Woodruff

    Seasons of Life - Hospice House in Woodruff is looking for a full-time Certified Nursing Assistant CNA to join our amazing team!  This position will be a part of our night shift team working 11:00 p.m. - 7:30 a.m. with rotating weekend and holidays.

    Ascension at Home provides comprehensive home-based services across Wisconsin. Our services include Home Health, Hospice and Home Infusion Therapy.   For their optimal comfort, patients and families are supported in their own homes whenever possible. For those times when it is not possible for end-of-life care to be provided at home, Ascension At Home has alternative homes specifically designed to meet the residential needs of individuals receiving hospice care such as Seasons of Life Hospice House.

    This position will provide safe, personal patient care at the hospice house including but not limited to:

    • Bed bath, tub bath, or shower
    • Weighs patient
    • Vital signs
    • Reminds patient to take medication as specified by RN
    • Dental, hair, and nail care, shaving, dressing/undressing
    • Elimination needs including toileting by offering bedpan, urinal, or taking to bathroom. (Includes catheter care, colostomy, urine testing/collection.)
    • Ambulating, transferring, positioning by using assistive devices such as walker, wheelchair, and prescribed ROM when taught by health care professional of Ministry Home Care
    • Provides housekeeping duties: meal preparation and cleanup, dusting, sweeping and mopping, vacuuming, stain removal, tidying patient rooms, recycling and garbage removal, doing patient’s personal laundry, dishes, and tidying the bathroom after bathing the patient
    • Feeds patients, maintains, intake/output as requested



    • Must be certified as a Nursing Assistant, passed the State Medication Assistant course, and be currently listed on the state’s Nursing Assistant registry
    • LPN/RN with current state license may also be considered
    • Current Driver’s License
    • Must be CPR certified


    • Prior health care experience is preferred

    Knowledge and Skills:

    • Computer skills required
