Summary

Work as a Scribe for Envision Physician Service! Provide scribe services in an Emergency Department at the direction of a Physician or Advanced Practice Provider (Nurse Provider or Physician Assistant) allowing the provider to spend more time with the patient, work more productively, and produce accurate and thorough documentation in real time. A scribe does not act independently, but documents the provider’s previously determined dictation and/or activities in the electronic medical record (EMR).

Responsibilities

Essential Duties and Responsibilities

  • Listen and take notes while accompanying the provider to the patient room or receiving verbal dictation
  • In the EMR, document accurate, thorough and timely physician notes that reflect the full complexity of care provided:
    • Document patient care activities performed by provider i.e. procedures, medications, conversations with consultants
    • Document applicable information from various sources i.e. hospital information systems, prior visits, nursing triage notes, EMS run sheets
    • Maintain awareness of conversations regarding patient care between the provider and other healthcare providers, patients, family, etc. to ensure applicable information is documented
    • Prepare discharge instructions or electronic prescriptions for provider review and signature
  • Ensure physician notes meet clinical documentation requirements for risk management, billing, etc.
  • Inform the provider of any incomplete documentation and seek out the information to complete the note
  • Monitor the status of the patient’s course, troubleshoot delays, and keep the customer informed of pending items i.e. waiting for x-ray to be done, all lab results back, etc.
  • Assist the provider in navigating the EMR
  • Identify and anticipate customer (provider) need and respond in an effective manner
  • Perform other duties as assigned

Working Conditions:

  • Ability to work day, evening, overnight hours and weekend and holiday shifts
  • Potential exposure to blood and infectious material
  • Potential exposure to radiation
  • Potential exposure to hospital violence

 

Qualifications

To perform this job successfully, an individual must be able to perform each essential responsibility satisfactorily. The requirements listed below are representative of the knowledge, skills and/or abilities required.

 

Education/Experience

  • Bachelor’s degree preferred (or verification that degree is in progress)
  • Biology coursework at the undergraduate level or above preferred; Anatomy and Physiology and Medical
  • Terminology coursework preferred
  • At least 100 hours of work, volunteer, or shadowing experience in the healthcare field preferred
  • Experience in a direct patient care environment preferred i.e. EMT, scribe, clinical technician in hospital, medical assistant in a doctor’s office
  • Basic understanding of medical terminology and general medical knowledge
  • Strong communication skills, written and verbal
  • Excellent listening and note-taking skills
  • Detail oriented with proven ability to work effectively under conditions requiring accuracy
  • Excellent organizational and time management skills
  • Ability to remain calm in a fast paced, at times stressful environment
  • Computer proficiency
  • Ability to type 60 words per minute or above, adjusted for errors
  • Courteous and professional demeanor/appearance

Immunization and Testing Requirement

  • Provide documentation of health information, testing, and/or immunizations including but not limited to: Tuberculosis, Measles, Mumps, Rubella, Varicella, Hepatitis B, Tdap, and Flu
